Requests for information can be submitted by completing the form below. Please allow 10 business days for OIERP to complete the request. In many cases, OIERP will follow up with you to discuss your request in more detail.

All requests must adhere to the guidelines set forth by the Family Educational Rights and Privacy Act (FERPA).
